In a small saucepan, mix the cocoa and sugar. Add 3–4 tbsp of the half-and-half and stir until you get a smooth, lump-free paste.
Pour in the remaining half-and-half and the brewed coffee. Whisk well so the cocoa dissolves evenly.
Warm over medium heat until steaming. Turn the heat down and keep it hot for 3–5 minutes; avoid boiling to prevent a cooked dairy taste.
Stir in vanilla if using. Divide into mugs; for a little foam, whisk or blend briefly, then serve right away.
