Set the oven to 150°C / 300°F. The low heat is what makes the long bake work.
In a large bowl, combine ground beef, milk, grated onion, and flour. Mix just until it comes together and holds shape.
Roll the mixture into walnut-sized balls. Lightly damp hands help prevent sticking.
Stir together ketchup, water, sugar, white vinegar, and Worcestershire sauce until smooth.
Place the meatballs in a baking dish in a single layer. Pour the sauce evenly over the top.
Cover with foil and bake for about 4 hours. Every so often, spoon a little sauce over the meatballs so they stay coated.
Let the dish sit for 5 minutes after baking, then serve hot with the sauce.
